Fill in your assigned affiliate number and internet affiliate link, copy and distribute.
Every form that arrives with your affiliate number and internet affiliate link
will be to your credit in the affiliate program. Also, if you are passing out at a particular event, fill that in.
Please inform us by email of who you are distributing to if you know his or her name,
or to what town or event you are distributing this flyer. All this will make sure we give you credit where credit it due.
